Efficiency Limit

IT functions are reported to be failing to meet the needs. Project failure rate is high. Schedules are slipping while we need to meet deadlines at an accelerating speed. Bugs and glitches jump at us from everywhere. Is there a reason for this or are we working in IT plainly incompetent?
